The SLV-AHEC has a long standing relationship with the Colorado Prevention Center (CPC).  Partners on the project include Rocky Mountain PBS, 9 Health Fairs, the CPC and the SLV-AHEC.  The SLV-AHEC has had kiosks placed at the Alamosa County Public Health Agency, Rio Grande Hospital in Del Norte and Dr. Brian Jackson’s office in Antonito.   SLV-AHEC rotates the placement the kiosks throughout the San Luis Valley.  Also, three Community Health Workers are employed through this program.  One Community Health Worker works from the Conejos County Hospital, one from the Costilla County Public Health Agency in San Luis, CO and the Community Health Worker is housed at the Alamosa Family Medical Center in Alamosa.  Their job duties are to screen individuals for cardiovascular disease, provide education, navigate individuals at high risk for CV disease to a “Medical Home” and provide follow up on these individuals.
